5Reimbursement for out-of-network services is based on the lesser of the dentist’s actual fee or the Maximum Allowable Charge (MAC). MAC refers to the Maximum Allowed Charge, which is based on the lesser of (1) the amount charged by the out-of-network dentist or (2) the out-of-network scheduled amount for the state where the dental service is performed.
