To qualify, complete these steps:

Enroll in the Black Car Fund Drivers Benefits Program.

  • Already enrolled? Download the app and log in to access your benefits.

Schedule your eye exam.

  •  Call us at 1-833-814-8590 to schedule your exam
  • After you are enrolled in Drivers Benefits, our Driver Support Team can help you schedule your eye appointment at a participating GVS in-store location. Our support team is available M-F, 8 a.m – 8 p.m ET. 
  • A vision test at the Drivers Benefits Vision Van does not qualify you for the Amazon.com gift card.

Get your eyes checked at a GVS in-store location.

  • You can get an annual eye exam2 and a pair of glasses or a 6-month supply of contacts3 at no cost to you using The Black Car Fund Drivers Benefits Vision Coverage.

Submit your receipt or exam confirmation before June 15, 2025 by using our easy form, and receive your gift card via email in two weeks or less.

  • You must complete your eye exam between May 1 – May 31, 2025, but you have until June 15, 2025 to submit your receipt or exam confirmation for your gift card. You will receive your gift card via email in two weeks or less. 

This offer is only available to eligible1 drivers who are enrolled in Drivers Benefits.

NOTE: To qualify for the $25 Amazon.com Gift Card, eligible1 drivers must be enrolled in Drivers Benefits and must have completed their eye appointment sometime between May 1st and May 31st 2025 by 11:59 p.m. ET at a GVS in-store location (and not valid at the Drivers Benefits Vision Van). One gift card per eligible1, enrolled driver. Restrictions apply, see amazon.com/gc-legal.
