Enroll + Get An Eye Exam in May = A $25 Amazon Gift Card for You!
May is Healthy Vision Month! To celebrate, our Vision Coverage provider, General Vision Services (GVS), is giving drivers who are enrolled in Drivers Benefits a $25 Amazon gift card for completing an annual eye exam in May at one of the many GVS in-store locations.
To qualify, complete these steps before May 31st, 2024:
1. Enroll in The Black Car Fund Drivers Benefits Program TODAY.
◦ Call us at 1-844-553-0511 and our Driver Support Team can help you – it’s EASY and FREE to enroll!
2. Schedule your eye exam.
◦ After you are enrolled in Drivers Benefits, our Driver Support Team can also help you to schedule your eye appointment!
◦ Remember, your exam must be completed in May 2024 to qualify for the gift card and the eye exam must take place at any one of GVS’ many in-store locations. (An exam at the Mobile Eye Care Center does not qualify for the gift card)
3. Get your eyes checked.
◦ Don’t worry, if you need glasses. You get an annual eye exam and a pair of glasses or a 6-month supply of contacts AT NO COST TO YOU using The Black Car Fund Drivers Benefits Vision Coverage.
4. Submit Your Receipt or Exam confirmation before June 15, 2024 by using our easy form and receive your gift card via email in 2 weeks or less.
◦ You must complete your eye exam between May 1 – May 31, 2024, but have until June 15, 2024 to submit for your receipt or Exam confirmation for your gift card.

NOTE: To qualify for the $25 Amazon gift card, drivers must be enrolled in Drivers Benefits and must have completed their eye appointment sometime between Wednesday, May 1st and Friday, May 31st 2024 at 11:59 pm at a GVS in-store location (and not valid at the Mobile Eye Care Center). One gift card per enrolled driver.
